Open the Hakeema URL provided by your administrator. Enter your work email and password.

Start a Session
Pick a patient — search the directory or add a new one. Choose the specialty so Hakeema structures the note correctly. If the patient has prior visits, their context loads automatically.
- Patient context shows known conditions, medications, and allergies pulled from the EHR.
- The AI uses this context to improve accuracy and flag conflicts.
Record the Visit
One tap on the microphone starts ambient capture. Speak naturally to your patient — Arabic, English, or both. The transcript fills the right panel as the conversation unfolds; the SOAP note builds itself in the left panel.
- Speaker labels separate Doctor from Patient automatically.
- Hakeema handles Modern Standard Arabic, Gulf dialects, and code-switching mid-sentence.
Review the SOAP Note
When you stop recording, Hakeema processes the audio in 10–30 seconds. The full SOAP note appears with every finding linked back to its exact source in the transcript. Click any field to edit.
- Hover any clinical item to see the line of conversation that generated it.
- Edit inline. The system re-runs code lookups automatically.
Verify Coding
Every diagnosis, medication, lab, and procedure is automatically coded against KSA-certified registries: ICD-10-AM for diagnoses, SFDA for medications, ACHI for procedures, LOINC for labs, SNOMED-CT for allergies.
- Codes are looked up deterministically — never invented by the AI.
- Confidence scores guide your review; low-confidence items are flagged for mandatory check.
Finalize & Submit
Once you've reviewed every section, click Finalize. The note locks, becomes part of the official record, and — if NPHIES is configured — the claim bundle is submitted automatically.
- Finalization is your professional attestation. Treat it like signing a paper note.
- All amendments after finalization are captured in the audit trail.
Hakeema is a clinical decision-support tool. Every AI-generated output requires physician review and approval. The clinician is accountable for the final record. Hakeema does not practice medicine.
